and questionnaires. If you are a new client, when you
speak with me on the telephone, before your first
session, I will ask you to fill out one or more of these
questionnaires and bring them with you to the office.
This will provide me with important information to use
in treating you, and will save us valuable time in our
session together.
To fill out a
form, please click on the link and print out the PDF
document that comes up. Then, fill out the hard copy of
the form with a pen or pencil.
